Intermediate Care Centre

Community 1st Sheffield is progressing a pathfinder project for health and social care services in Sheffield. Although it is still early days, Community 1st is fully involved in the integrated approach between NHS Sheffield, Sheffield City Council, adult social services, Sheffield Teaching Hospitals NHS Foundation Trust and GPs in the development of this new model for the delivery of care.

A commissioning board has been established with representatives from the above to develop the commissioning and service proposals for this exciting new development.

This development for Sheffield will set out a ‘Health’ pathway for people requiring intermediate care i.e. care for patients who require nursing care.

As part of the model it is envisaged a 120-bed facility, equivalent to the current number of intermediate care beds provided across multiple sites throughout the city will be required. However, the length of patients’ stay will be reduced by 50% from its current level with the new centre.

The preparation for this has been based on the ‘Pathways for Intermediate Care in Sheffield’ report written by Dr Tom Downes, a Consultant Geriatrician at Sheffield Teaching Hospitals who is involved in the planning of the model and the new centre.

As well as the in-patient bed facilities, services are likely to include therapy, specialist nursing, x-ray facilities, podiatry, ophthalmology and rehabilitation services, and a gym. 

The Centre would also provide the base for the multidisciplinary teams providing support to patients in their own homes.

A site options appraisal has been undertaken, though as yet the site for the new centre has not been confirmed.
